| There are several forms for facilitating the practicum process and ensuring a smooth transition from the practicum to the thesis. The forms listed below provide a mechanisms for us: 1) to gather information which enables practicum faculty and staff to advise and assist students in locating a suitable practicum match; 2) to document the practicum and thesis data arrangements that are sometimes made many months in advance; and 3) to evaluate our practicum process.
We currently employ two software packages to facilitate the electronic completion of surveys. The Practicum Survey Form is in SurveyMonkey which can be completed directly on-line and submitted without downloading and saving it to a computer. All other forms are in Adobe (PDF) writable format. These forms must be completed, saved and then submitted by email. To use an Adobe PDF writable form, click on the appropriate link, download the form to your computer, complete the form electronically by typing directly into the form, save it on your computer, and e-mail a copy of the completed form to jp376@columbia.edu.
To save your typewritten text in these forms, you must be using Adobe Acrobat Reader Version 7 or later. If you are having trouble saving a completed form, check to ensure that you are using Adobe Acrobat Reader version 7 or later. You can obtain the latest version as a free download from Adobe Corp., the maker of Acrobat. The Practicum and Thesis Data Checklist lists the major steps for completing a domestic practicum, an international practicum, and for applying for practicum equivalent experience. It also has an item for the student to indicate their status in securing data to be used in completion of their thesis. Information related to the availability of thesis data (Question 16) should submitted no later than the first week of the third semester.
The Practicum Agreement Form is completed after finalizing the selection of a practicum site but prior to beginning the practicum. It documents practicum arrangements, including whether thesis data will be available to the student.
The Practicum Evaluation Forms are completed by the student and by the practicum site supervisor at the conclusion of the practicum. Although information from these forms is summarized for evaluation of the overall practicum program, individual responses remain confidential.
The Zena Stein Excellence in Mentoring Award Nomination Form is to accompany nomination materials for the award. The nomination form can be completed and submitted electronically by the mentored student or a co-worker of the mentor or via mail. |
